DalMont Phoenix Foundation lodgers rights are as follows: 

a. To be treated with dignity and respect. 

b. To participate actively in your recovery. 

c. To be given information regarding informed consent prior to the start of your stay. 

d. To be seen by a private physician with the understanding that all costs will be the responsibility of the resident. 

e. To have all information pertaining to stay held in confidence. 

f. To receive information regarding costs. 

g. To be fully informed at the time of admission of the rights and responsibilities set forth herein and of all the policies and procedures along with the phasing system guidelines. 

h. To initiate a complaint or grievance procedure and understand that you may begin the process by filling out a grievance slip or contacting DalMont Phoenix Foundation. 

i. To request referral resources in the event of your dismissal from DalMont Phoenix Foundation

 j. To not be required to perform services for DalMont Phoenix Foundation

K. DalMont Phoenix Foundation will abide by Wisconsin state confidentiality and privacy law, including WI statute 995.50.
